Release and Waiver

In consideration of the exposure afforded me by participating in the production and distribution of video programming (“Program”) by Bozz Lady Productions, LLC, Inc. (“Lenesia Cooper”), and for other good and valuable consideration, the receipt and sufficiency of which is hereby acknowledged, I hereby grant to Bozz Lady Productions, LLC, its successors, assignees, licensees and designees, the irrevocable and perpetual right, but not the obligation, to use, as Bozz Lady Productions, LLC may desire, all still and video and soundtrack recordings which Bozz Lady Productions, LLC may make of me and my voice, including the right to use my name and likeness, including photo, video, and/or film likeness, in the Program, and in connection with advertising, publicizing, exhibiting and exploiting the Program, in whole or in part, alone or in combination with other programming, in any manner whatsoever and at any time, by any and all means, media, devices, processes and technology now or hereafter known or devised in perpetuity throughout the world. This grant includes without limitation the right to edit, mix or duplicate, and to use or re-use the Program, in whole or in part, alone or in combination with other programming, in any manner as Bozz Lady Productions, LLC may elect. I acknowledge and agree that Bozz Lady Productions, LLC, its successors, assignees, licensees, and designees shall have complete ownership of all programming in which I appear, including copyright interests therein, and have no financial commitment or obligations to me as a result of this agreement, and that the rights granted hereunder are not subject to the requirements of any collective bargaining agreement. I represent that I have no interest or ownership in such programming or its copyright, and that I have the ability to enter into this agreement. If I am under eighteen years of age, then my parent or legal guardian agrees to all terms and conditions of this agreement and acknowledges such agreement by signing below. I hereby release, indemnify and hold Bozz Lady Productions, LLC, its officers, employees, shareholders, directors, agents, successors, assignees, licensees, and designees harmless from any and all claims, demands, liabilities and expenses (including, without limitation, reasonable attorneys' fees and costs) arising out of or in connection with such use including, without limitation, any and all claims for invasion of privacy, infringement of my right of publicity, defamation (including libel and slander) and any other personal and/or property rights, and I agree that I shall not now or in the future assert or maintain any such claim against Bozz Lady Productions, LLC, its officers, employees, shareholders, directors, agents, successors, assignees, licensees and/or designees.  I understand that Bozz Lady Productions, LLC has been induced to proceed with the production, distribution and exploitation of the Program in reliance upon this agreement. I acknowledge that I have not entered into this agreement in reliance upon any representation (written or oral, express or implied) of Bozz Lady Productions, LLC not contained herein. I agree that Bozz Lady Productions, LLC may assign this agreement and its rights hereunder to any third party. This agreement shall inure to the benefit of and be binding upon Bozz Lady Productions, LLC’s respective successors, licensees and assignees.

By signing this Agreement, I assume all risk participating in the aforementioned activity. I acknowledge that the event production may post some risk of personal injury and that I undertake and assume this risk. I further waive and release Bozz Lady Productions, LLC, its successors, assignees, licensees, and designees, from any and all liability, including, but not limited to, liability arising from negligence or fault of the entities or persons for any injury or disability which may occur as a result of my participation in the above production. I am assuming all risks on behalf of myself that may arise from negligence or carelessness on the part of any of the persons or entities being released, as well as from property that is owned, maintained, or controlled by the above-named entity, Bozz Lady Productions, LLC, its successors, assignees, licensees, and designees.
